SANTA CLARA, Calif. - The Santa Clara University women's tennis team will hold walk-on tryouts the first two week of classes for any student interested in joining the team in the fall.

Each student must complete and turn in the following forms on the first day of tryouts:

Student-Athlete Forms

Insurance Declaration
Athlete Physical
Athletic Waiver and Release of Liability
Medical History Questionnaire
Orthopedic/Neurological Exam
Athletic Accidental Medicare Coverage
Authorization for Disclosure of PHI
Notice of Privacy Practice
