SANTA CLARA, Calif. - The Santa Clara University softball team will hold a meeting on Wednesday, Sept. 24, for any SCU students interested in trying out for the team. A two-day on-field tryout the following Monday and Tuesday, Sept. 29-30, will follow.

The mandatory meeting will be held at 3 p.m. in the Castruccio Room behind the west side bleachers in Leavey Center. Interested students should bring their fall class schedules to the meeting.

Each student must complete and turn in the following forms on the day of the meeting:
Student-Athlete Forms

Insurance Declaration
Athlete Physical
Athletic Waiver and Release of Liability
Medical History Questionnaire
Orthopedic/Neurological Exam
Athletic Accidental Medicare Coverage
Authorization for Disclosure of PHI
Notice of Privacy Practice
